Privacy Policy & Consumer Health Data Privacy Policy

- Last updated
We know that when you come to us, you’re trusting us with information about your family. This policy explains, in plain language, what we collect, why we collect it, who we share it with, and the choices and rights you have. It also serves as our Consumer Health Data Privacy Policy under Washington’s My Health My Data Act (RCW 19.373).
1Who we are
NK Lice Solutions LLC (“NK Lice Solutions,” “we,” “us,” or “our”) is a head lice clinic located at 811 W Yakima Ave, Suite 102, Yakima, WA 98902. We offer head lice screening (head checks), comb-outs done by hand with a pesticide-free dimethicone conditioning oil that helps the comb glide, and an in-clinic hair wash. Our lice screening is not a medical diagnosis.
Most of the people we help are children who are brought in by a parent or guardian.
This policy applies to information we collect at our clinic, when you book or communicate with us, and through this website.
2Information we collect
Depending on the services you choose, we may collect:
- Parent or guardian contact information, such as name, phone number, and email address.

- Hair type.
- Allergies and sensitivities.
- Scalp conditions.

- Screening and treatment results.
- Retail purchases.
- Signed consent forms.
- Text and email reminder preferences.
- Before and after photos, only if you give opt-in consent.

4Where we get information
We collect information directly from you, the client, or from the parent or guardian who brings a child to us. This happens through intake forms, when you book an appointment, and when you contact us by phone, text message, or email.

10Children’s information
Most of our clients are children. Information about a child is provided by the child’s parent or guardian. We do not knowingly collect information directly from children online.
A parent or guardian may exercise the rights described below on behalf of their minor child.
11How long we keep information and how we protect it
- Paper records are kept in a locked file at the clinic.
- Digital records are handled through Square.
- Access to records is limited to staff who need it.
- We keep records for at least 3 years. We keep children’s records until the child turns 21.
- If you ask us to remove before and after photos, we remove them within 7 days of your request.
12Your rights
Washington residents have rights under the My Health My Data Act, and we offer these same rights to anyone we serve. You have the right to:
- Know and access. Confirm whether we collect or share your consumer health data, and get access to it, including a list of the third parties it was shared with.
- Delete. Ask us to delete your consumer health data. When we delete it, we will also tell our service providers to delete it.
- Withdraw consent. Withdraw consent you have given us for collecting or sharing your consumer health data.
A parent or guardian may make these requests on behalf of their minor child.
We may need to keep some records when we are legally required to, or when they are needed for insurance or legal claims. If that applies to your request, we will explain what we kept and why.
